Please add a description to custom fields to allow for instructional tooltips for users

Problem Statement:

We have custom fields in use, but some are hard to explain to users how to fill them out with just the name/label on the form.

Proposed Solution:

Please add the ability to supply a description for a custom field that can be displayed to the user either A) with a tooltip that appears when a user hovers over the field label, or with a small info icon (i) or help icon (?) next to the field label that can be clicked.

Support for rich text in the description would be nice.
